Apple TV's Cape Fear has delivered a major surprise in episode 3, with a star closely associated with the 1991 film making an unexpected appearance.

The latest episode of the psychological thriller continues to follow Javier Bardem's Max Cady as he tightens his grip around the Bowden family, having re-entered their lives following his release from prison.

But episode 3 – titled 'Phantom Sensations' – introduces a new mystery, as Cady becomes the target of someone else's attention.

Throughout the episode, Cady is watched by a mysterious figure, whose presence appears to unsettle even him. The same figure also shares an eerie encounter with Anna Bowden (Amy Adams), raising further questions about who she is and what she wants.

It is only in the episode's closing sequence that her identity is partly revealed, when Cady receives a disturbing video message.

In the clip, the woman serenades him with a version of 'Jingle of a Dog's Collar' by Butthole Surfers, adding another unnerving layer to the mystery after Max also receives a gift of a dog's collar.

Having spent much of the series so far as a smooth, magnetic and calculated presence, Max breaks down into tears before flying into a hysterical frenzy and smashing the TV playing the message.

The final twist? The woman is played by Juliette Lewis.

Juliette Lewis’s Danielle Bowden clings tearfully to Jessica Lange’s Leigh Bowden in a rain-soaked scene from Cape Fear (1991), as a hand reaches in from off-screen and the pair look terrified.

Lewis, of course, has a major association with Cape Fear, having starred opposite Robert De Niro, Nick Nolte and Jessica Lange in Martin Scorsese's 1991 film version.

In that version, Lewis played Danielle Bowden, the teenage daughter of Sam and Leigh Bowden, and earned an Oscar nomination in the best supporting actress category for her performance.

For now, the series has not fully explained who Lewis is playing, what her connection to Max Cady is, or why her message provokes such an extreme reaction from him.

But the closing moments of episode 3 suggest that whoever she is, she knows exactly how to get under Cady's skin – and may have a hold over him that no other character has yet managed...

Lily Collias, who plays Danielle's counterpart Natalie Bowden in the new Cape Fear series, recently told Radio Times that she had been able to speak with Lewis while preparing to play the role.

"I had the pleasure of being able to connect with her," said Collias. "She was honestly very, very much a guiding light about all of that, because she was like, 'This is entirely your own, and you're gonna make it amazing.'

"Her making those stepping stones for me made it not overwhelming."
